Post Discharge Support

We have an additional registration for domiciliary care. This will enable the Allag Care staff team to transfer the care package to Brand Care Domiciliary care team which is based in separate offices at Gwendolyn Road.

They will provide individuals with the support they need and ensure a smooth transition into the community. Individuals will be supported by support staff with whom they would have built a rapport with if they are able to return to live independently.
