Refferals

Referrals to Gwendolen road will be via the Multi disciplinary team, care managers, CPN, social workers, RNLD and private referrals. Through the Pre-admission assessment we will assess the needs of the individual and ensure that we can provide appropriate service to the potential individual prior to the admission.

All funding and costings are on an individual basis of the individual needs. The funding must be agreed prior to any referral being formally accepted and agreed.

Care packages will be developed with the involvement of the individual, family and other professionals involved in the care needs of the individual. The care packages are developed to ensure the needs of the individuals are met and a consistent person centred approach will enable the individual to enhance their quality of life and ultimately successful intervention. Pre admission and Phased admission is available with visits and overnight stays are available prior to the individual admission to Gwendolen Road Care Home
